Hi all.

I'm looking for some advice. I have a multi entry Schengen visa which was issued by Portugal last year.

It's still valid, so I'm wondering if I can use it again but enter Italy from South Africa.

Will that work or would I need a visa issued by Italy?

Just make sure that you comply with all other conditions besides visa for entry, like meeting invite for business etc.

So make sure to carry all the documents I'd normally take for a visa interview, right?

According to: https://www.visahq.com/schengen-visas/schengen-visa-multiple-entries

Schengen visa valid for 3 years, multiple entries

Those who have received and legitimately utilized a one-year MEV during the last two years are eligible to apply for the three-year MEV.

The bearer of this visa may visit the Schengen Area as often as they want for three years. Even with this visa, however, a visitor may spend up to 90 days in the EU in any 180 days.
