Cant see if you reinstalled GPU drivers from scratch, thats where i would start.

Most people use Display Driver Uninstaller (DDU).

So @|tera| suggested not to use 3rd party apps like ddu, however I didn't reinstall anything, just went back to nvidia control panel and forced the frame cap from there and started testing v sync on and off from nvidia control panel and the game settings. Testing all the different combinations was a pain as changes only took effect after closing and reopening the game.
