SIP trunk? What is it?

In very simple words, it is a tunnel established over the internet or MPLS VPN network between company's PABX and VoIP providers infrastructure in order to make cheap VoIP calls

Obvious reason for this is mitigation of telephony costs

Do you have to have one to make voip calls?

If you mean PABX, the answer is no. You don't need a PABX.

If you only require one line, all you need is a VOIP-enabled phone and a SIP provider, for home use you can use www.freshphone.co.za signup and they provide all the settings for your SIP phone. You then have to enter the settings on the phone, similar to how you setup an ADSL/WiFi Router and you good to go.

You can also use a software based SIP client, on you cellphone, example on Android you can try CSIPSimple
 
So, if I have an onsite pabx, and want to use voip, I need sip trunks? If I am going with a hosted pbx, I simply need internet and handsets/soft phones?

Pretty much.

Just make sure you prioritize the voice traffic or have a dedicated link for voice only.

You pretty much had it. In simple building block terms it may looks something like this:
Onsite: Phones > Onsite PBX + SIP Trunk > Dedicated internet link > VoIP Provider breakout
Hosted: Phones > Dedicated internet link > Hosted PBX + SIP Trunks > VoIP Provider breakout
