Telkom nightmare

Can anybody please help resolve a long standing problem I've been experiencing with Telkom? Even though I've cancelled my account in desperation, new SIM numbers are still being activated on my account months later and I'm still being billed for services I haven't used. I'm even being threatened with blacklisting!
I've tried everything but cannot find anyone at Telkom willing or able to help. Is there an Ombud who deals directly with Telkom issues? What I mentioned here is just the tip of the iceberg.
