If I understand correctly, he seems to be warning that playing trills with the third and fourth fingers, or with the fourth and fifth fingers, can damage the tendons in your hands.

My right thumb joint, where my thumb connects to my hand, is painful when I squeeze it, although it feels good when I massage the joint. I believe I may have injured the joint from overuse or incorrect technique. I believe that playing trills using your thumb may not be healthy.
