tire blowout question

Hey guys , stressful afternoon.

Was driving home when i had a tire blowout on the highway . As i felt and hear the noise , i pulled over to the side and saw the wall of the tire cut and smoke coming out of the tire . since i was on a hill i had to roll down about 200m to get to a flat surface . The car was on an uneven shoulder but i manage to jack it up put on the spare , and continued on the journey . Started feeling vibration so i went of the highway , and a little while later someone signaled me to say my tire is wobbling .

Jumped out and tightened the nuts. its a Toyota so it has nuts not the bolt system ,drove another 2 km , jumped out to check , and the nuts were getting loose again , nurse the car back home , tightening the nuts which were getting loose . 3 of the 5 were getting loose, are two slightly loose.

Got home just jacked the car up now , and the hub feels very hot compared to the other side ,
check for fillings on the nuts and they dont really seem to be stripped .

any ideas what had messed up here? Taught maybe i didnt put tire on correctly due to the angle the car was when i changed it perhaps ?

Hub Damage ? Change nuts ?

Something my dad taught me...

Could you have fitted the nuts incorrectly?
The tapered side must face the rim.
 
Some cars have different nuts for the steel spare wheel. Check in the emergency tool kit if there isn't a bag of nuts to go along with the spare wheel.
 
man im a real cake, went back to it now , discovered there was a black spacer (sprocket ring ) removed it , re seated spare tire , took it for a test drive and all is fine .
