UPS Flight Crashes In Louisville

It's the one with the springbok livery on it at some stage. Also from the late 80s. ZS-OAF. Not sure if that was on purpose but its "Old as F..."

ZS-OAF (B737-400) was previously with BA/Comair, then went to Kulula, painted in its 'Camouplane' livery (both liveries between Mar 2007 to Aug 2017), before going over to FlySafair, where it was painted in "For the Love of Flying" special colours between Jun 2018 - Feb 2023, and was then repainted in the current revised FlySafair livery...

The families suing are those of Angela Anderson, who was killed while she was shopping near the airport, and Trinadette Chavez, who was working at an Auto Parts dealer when the aircraft crashed.
 
NTSB update confirms that the 2nd (rear) engine also failed, due to debris from the 1st engine. The engine mount failure was a known issue that Boeing downplayed:

FedEx is preparing to return its grounded MD-11 freighters to service after a six-month ban. Technicians will be deployed to 16 locations worldwide to remove engine pylons from 29 aircraft, with parts sent to Indianapolis and Memphis for installation of redesigned Boeing bearings.

FedEx plans to restart MD-11 operations with two aircraft by late May, pending Federal Aviation Administration approval. MD-11 pilots will also complete a 3-day refresher training course.

 
The U.S Federal Aviation Administration has approved Boeing’s protocol for safely returning MD-11s to service.

The FAA said ⁠after an extensive review it approved Boeing’s protocol for returning the planes to service that includes maintenance and inspection requirements.

FedEx said it plans to resume operations of its 28 MD-11 aircraft later this month once the planes meet Boeing’s maintenance requirements.

NTSB Investigative Hearing: UPS Flight 2976 Crash During Takeoff - NTSBgov​

The National Transportation Safety Board held a two-day investigative hearing May 19–20 into last year’s crash of a United Parcel Service MD-11F cargo airplane shortly after takeoff from Louisville, Kentucky.

The three crewmembers aboard the airplane were killed. Eleven people on the ground were killed and another 23 injured.
