Is is possible to use a isp that is not situated in SA? Or does the beast not approve?

No and afaik its not possible anywhere in the world to do that.

It is possible

It is possible, I use one, but you will always be using an SA upstream provider - the costs are obviously higher too. I use CGNet's travellers access service with Ipass - I have a HUGE international phone book to choose from - wireless providers, dial up, ISDN, etc. etc. Very useful for when I'm in Atlanta, Dar Es Salaam or Johannesburg (airport, hotel, etc.)
All in all it's a nice service if you travel alot, but pointless if you don't. AFAIK Verizon business has a similar international dial up setup - i.e. verizon customers can dial up from many different countries using their accounts.

My mistake - seeing the post was in ADSL ISP Discussions I assumed he was talking about ADSL access.

I use iPass as well but as you say you're still using local providers and paying a premium for it. With the prevalence of broadband in public spaces, hotels, coffee shops etc overseas we're looking at dumping it now
