I am a Foreigner. In my travels if I see someone who needs help, I stop
and help. There does not seem to be a "medical professional" available.
What exactly do you expect? The OP (that's you) pastes a photo but doesn't bother answering any additional questions.

I stopped to help because it looked like she had no options.

If this is a common disease then everyone here should know
how to recognize it and be able to help her, so she does not
have to depend on a Foreigner.

Chiggers was the best guess, so far, and the red color on the photo
could well be a treatment that will do in whatever local version
of chigger that might be about.

The answer to south africas health system is to leave the big health
companies to do their business.

The government, should then do a separate "safety net" that is clearly
not as good. It could then be made better by making medical care
available at pharmacies. Doctors would still be at the top, but you
would enlist people all the way to the street to do medical care. You
would literally have motorbike drivers as health care providers.
 
It's a Mango fly. They lay their eggs on damp clothing. The eggs get in contact with the skin. They turn into little worms which burrow into your flesh eating away.

After a while the get bigger upto 6 cm long and a centimeter wide.

Munching away the patient feels sharp stabbing pains.

If the worn does not be cut out it burrows its way into the brain and munches away until it grows into a 2 meter monster, eating you from the inside out.
Then you die.

many people. Cost, Distance, and Cold Reception are just some
of the impediments.

So if your medical plan is working, stand up and cheer. And let's
keep your care providers in business, and undisturbed.

Her foot requires a medical plan that "delivers" and that would be

@Foreigner the problem is how you approach an issue and the lack of information you give. You want help with something you need to give more information and context.

From what you have given us, a picture and there you go. The person needs a doctor clearly.

If the problem is that there are no doctors you need to address that issue. Posting photos that has no context is not going to help you or anyone else.

I am trying to help a person who is sick and has no options.
Some posters have already helped this girl. I told her about
the mango fly idea and she agreed that the black spot in the
middle of a sore is something that she noticed also.

So nobody has practiced any medicine and she is getting much better.

There is a great story of South Africans coming together to help
one another, and I would like to tell it. Put the mods away.
Send Red Devil to troll elsewhere. And thanks Urist for returning
us to the topic of Helping Others (with a bit of added sensitivity)
