Who sets concurrent sessions limit?

Hi Guys,

I want to know who sets the concurrent sessions limit on an account? Is it the network provider or ISP/reseller?

I have an IS account through a reseller which worked great up to last week when I could no longer establish a third concurrent session on it.

Also, what does "foreign host close tcp" mean as indicated in the log?
