So this week I just had it, I had the entire global catalog not being able to see the active directory and the entire DNS was wiped out and the backup tapes were two days away, and this was @ 2am and the staff was about to arrive @ 5am.

Got it sorted!

I have remotely locked myself out of a firewall that sits in the middle of nowhere, the change also stopped all traffic flow. Downtime, 3 hours
and
i have started a process on a firewall, which cant really be termintated, but stops traffic flow until its done. This was on a big auditing firms firewall, during peak traffic times. I cut them off for 45 minutes, no change control.
 
Where to start: had it all flooded data centre with no option to shutdown and move servers, blaster nuking couple thousand pc's and millions of $$$ at stake, one of my underlings chose to shut down instead of log off critical management server affecting support services for multi-billion $$$ company and the list goes on and on... yep good times :(
