I was wondering if anyone who has done the procedure could fill me in as to what the 'Unit Model Number' is
that is on the form to fill in? All I have is the serial number and the manufacture date and the product ID. Suggestions
and help most appreciated.

Leave the "Unit model number" blank. And emailing your form to them is way more quicker than fax.
