There is definitely a list of exchanges posted somewhere here on MyBB, or somewhere on our local internet, I've seen it a few times before. Try a Forum search or Google search. Let us know if you find it.
The list provides the dialing code + name of exchange, where the name corresponds to the area of the exchange (e.g. Lenasia, Lawley, Rosebank, Sandton1, etc). I don't think there are physical addresses for the exchanges, but once you know which exchanges you're interested in, you can phone Telkom and ask them where it is located, or ask around on these forums, and someone will be able to tell you the location.
You might also want to try visiting the Telkom Office / Telkom Direct Store in the areas you are interested in moving to, and asking them where the exchange is.

You can search through here: http://www.broadbandstats.co.za/exchanges/province/Gauteng - if you click on the exchange code it gives you a google map of the location.

Thanks for posting, exactly what I was talking about. I see they've updated the site to include Google Maps support. Pretty awesome. However, the location of a few of exchanges that I know of are not 100% accurate on the site. Also, the info for 1 exchange I know is incorrect. It's marked as does not support ADSL, when in fact it does. I've sent them an email through the site to update the coordinates and information.
