Android - form / data collection software

Once upon a time I thought this kind of software was pretty common on Android, until I had a need for it and tried to find one that suited me. What a schlep.

Has anyone had experience with a basic survey type data collection tool for Android? Something that works offline as well as online would be fab (though offline is required, online is a nice to have).

What would be really helpful is if anyone who's actually tried something that works could share their experience.

All the while, through a live connection between your computer and your phone, your app appears on your phone.

So I need always on connectivity? I'm going to need to collect data away from any form of internet...
or am I misunderstanding

That is for the dev/testing section, you can roll out apps to run on the phone without the need for a PC.
