Android Software for mapping potholes?

Looking for some ideas or advice for a project I am working on.

I want to do a complete empircal audit of service delivery issues in my area. I want to map every pothole, broken pavement and such onto a public google map (with photos of all service delivery markers) so I can get a holistic overview of the scale of the service delivery issues in my ward.

I was wondering if anyone has come across good apps or recommends an app that could easily do this. Im going to be working with teams of volunteers, many of who will have limited technology skills. So I would like a simple app, where the volunteer just has to take a picture of the pothole, put in a label, select from a drop down list of issues and click send. GPS data should be automatically captured with the image. This would then connect to a shared map (ideally Google) so it can be shared with other people in my area.

What would you recommend I look at to achieve this. I had a look at GIS cloud, but it is $20 a device :/

Program your own app? Then use JSON to store the data on a server, and then proccess said data

Im looking for the non "build your own" option :D I do not have free time to learn to program in android at the moment.
