Correct you will probably need purchase something for get it work.
We settle with a VM at end of the day, devs said also need this from some scripts they are running.

Setup a public endpoint. (seems you don't need gateway then)
Then move the database from Managed Instance to Azure SQL DatabaseYep. Just what I didn't want to do - see OP
Or cough up money for the subscription for the gateway
