2005. Liverpool won the champions league, finished outside the top 4 in England, that meant the holders of the competition wouldn't be in a position to defend their trophy. UEFA allowed 5 nations from England in that season ( Chelsea, United, Arsenal, Everton and Liverpool). Unfortunately, or maybe not, Everton lost in the qualifiers and didn't make it to the group stage, if they did England would have been the first country to field 5 teams in a single season of the champions league.
