What the hell is GitHub? On 2nd thoughts, don't bother to answer, I'm not interested anyway.
 
Yes, had to way back when I started at my current company. Since then I moved to our own visualstudio.com one.
 
Yes. Slight chance our company will move to Git from svn.

You don't (necessarily) need a github account for that ...

I have implemented (for previous $job):
* Normal git with cgit for easy viewing (for simpler projects with no CI etc.)
* Self-hosted (community-edition) gitlab (with poor-man's geo-redundancy), which is very nice for more complex projects with GUI for code reviews, CI integration etc. for projects that couldn't run CI using 'cloud' CI tools. I think we started using this for a production code-base with gitlab-ce 8.4 or so.

I have a github account for contributing random fixes to software I use that is mastered on github. I have never used a commercial github account (or github enterprise).

I also have a gitlab.com account, and accounts on a few other open-source project hosting sites or projects that have their own svn and/or git infrastructure.

At work we git with other tooling.

For personal use I just use stand-alone git pushing to my 'NAS'.
 
I would not be able to live without my Github account, literally, I wouldn't be able to submit any work and get paid :D
 
Yes, currently only with public repos which remain because my code is now forked into other projects and being used as part of Linux. Shut the private ones down when the Microsoft takeover happened.
No company or proprietary code on github anymore
