Documents required for Vodacom contract

Hi Guys

can you please tell me what documents are required for a Vodacom topup contract?
 
Provide proof of address (a utility bill or other account), 3 months bank statements, a salary slip to prove that you're financially sound and able to meet your monthly cell phone payments, as well as your ID.
 
As per JeanetteR (did a new contract in November) and you also need to remember they only do debit orders now.

I just took one out this morning:
1. ID
2. Letter head with order on it
3. Utility is only needed if you have not been rica'ed before

I took the My Gig 1 @ R199pm x 24 months and got a free 1TB External USB Hard Drive

I know this thread is quite old but does all this still apply? I want to get a vodacom contract by the end of the month (September 2014) and I am not really sure what documentation is needed?

Do I still only need the following:
1. ID
2. Proof of Residence (Is an electricity bill fine?)
3. Recent payslip

where does it state we have to produce bank statements, these are highly confidential documents, FICA doesn't even require them
