The GSMA royally fecked up the 5G protocols, just because you’ve got a 5G icon doesn't actually mean you have a 5G connection. It’s likely you were still connected to LTE, but the cell you were connected to is capable of 5G but it isn’t active.

The GSMA recently recommended that phones show a 5G icon even when they can't find 5G NR coverage. Thus, it's clear that the relative simplicity of today's mostly ubiquitous '4G LTE' status icon is not going to carry over into the 5G future.

Basically three 5G icon states should really still be 4G.
I went Durban and Capetown for business trip, found my iphone 12 with sim is with 5G, but Durban speedtest was really bad, just like 3G, Capetown waterfront 5G somewhere. also bad speedtest, 4G in Johannesburg is better

Easiest way to tell if you’re actually on 5G is to use an app like Ookla Speedtest, the app can tell whether you're still on LTE even if a 5G icon is showing, it shows 5G or LTE next to the phone and network name on both the test screen and results page.

The point being made is for any network, the 5G icon doesn’t necessarily mean the phone is using a 5G signal, (I already explained further up the scenarios when this can happen).
That’s why you can have a slow LTE Speedtest even though the phone is reporting 5G. Ookla Speedtest will show whether its a true 5G signal and not just LTE masquerading as 5G. I used one of my Speedtests as an illustration of where to look.
