I don't know if this has been answered before, please forgive me if I am asking the same question that has been asked before.

What is the maximum distance does your premises have to be from the ADSL enabled exchange, I see some sites saying 5.5Km, others state about 3miles, which equates to about 4.8km ?

As I have telskum technicians on my remote site stating that my offices are to far from the exchang, i.e. 4.2km

You should be fine. It depends on the cable length though.
Although you may be physically that far from the exchange, the cable routing could be a lot longer.

Slip the guy a twenty and you'll suddenly be in range. ;)

If the techie says youre 4.2km then you should be alright - AFAIK they're measuring the cable length and not the line of sight distance. Perhaps the line quality is very poor.

The furtherer away you get the worse the signal gets. Worst case senariio go for a slower package. The slower the dsl speed the more tollerant it is to interference and distance.
