Even when antibiotics are used judiciously this happens. This is an issue in hospitals with nosocomial infections.

Same happens when people don't complete their courses or have poor absorption (you take your med but it doesn't get into the bloodstream in high enough levels from your gut) or other medications up-regulate (speed up) the breakdown of antibiotics as can happen through the cytochrome p450 hepatic enzyme system.
