Front I/O Panel

Where can I get a front I/O panel similar to these products:

I needs to have a couple of USB ports and a SDHC card reader.

Only other one I could find with USB and SDHC was this one:

Otherwise there is also the Sony MRW620 which seems to have SDHC, fits in a 3.5" bay and has one USB port available. It's listed as having the following: CF, SD/SDHC/MMC , SM, micro SD , MS/ Ms Pro Duo/ M2.
