For extreme cases - doctor for injection - voltaren I think.
My attacks seem connected to dehydration - I try to keep well hydrated now. My attacks started when I visited Thailand. I think the tropical heat and lack of hydration plus lots of walking and, funnily enough, beer triggered it. Now I just drink lots of water - I don't avoid any specific food but I drink far more red wine than beer. Beer definitely seems to have some affect for me.
Gout runs in the family, but I will try to avoid going on chronic meds for as long as possible and I haven't had a proper attack {touches wood} for well over a year now.
