Those are very serious symptoms and are neurological in nature. Your best bet is to see a proper physician and if he cannot diagnose you properly then you need to ask for a referral to a neurologist, if he doesn't refer you by his own accord. Don't mess around with this because your mommy likes the GP you see. Get it done now!
Maybe you need to be scared into sorting this out properly? If so, the problems you describe are symptomatic of a brain tumour, ischemic attacks (mini-strokes), chronic anemia, heart problems, lung problems, hypotension, or simply an ear infection or blood glucose problems. The symptoms you describe are those that any physician would consider critical and the duration of 5 months means that diagnosing it would become a matter of severe importance.
Now stop screwing around by asking a forum of nerds and go and see a proper physician. This is not the place to ask for medical advice, be it minor, or severe (as is your case)...
