It is clearly incompetence and not greed. If they were competent enough to identify the problem, they would have entered into a very expensive maintenance agreement with kickbacks and not both, as greed would have resulted in them neglecting maintenance to save money, but the return on spending the money is higher.

I didn't see that coming. My bet was on over pressure (explosion) that isn't visible, only instrumented. So to ignore actual visible system failure seems truly incompetent in a Dunning Kruger scenario in that those running the plant couldn't even comprehend what they were seeing (dust and visible build up) and what the ramifications of this would be.
