Technically they can instantly change it. But from my own experience, it's very rare that they do so. And instead you are made to wait days or even a week or more.

If from 2 -> 4Mbps, yes.
As you know, it would depend on the DSLAM, IMAX, MSAN or ISAM that you are connected to...and from 20 -> 40 in my experience.. about 3 mins to upgrade the sync.

If on an old DSLAM (4Mbps maximum) and you want to upgrade to 10Mbps, a techie will have to go out and physically change your connection to a different DSLAM, etc. in the exchange. So it won't be immediate.
