So how do they verify that their contention ratios are still valid? Or if ther backbone capacities are still okay? Or, if servers etc are overloaded? Or a dozen other things equally important to running an ISP properly?
At bulk level. You don't need individual usage to measure that, you can look at your interface to transit and peers (and network operators)

So they do measure usage, but just not at the individual level?
How does an operator know if his contention ratio policy is delivering what it was intended to deliver? Or don't they care? Is it a case of our contention ratio has been set and we don't worry if, in the end, the customer suffers from poor throughput at times?
 
The novalty of how much did I use wears off, dont worry about it.

Correct. I said they don't measure your usage. Not overall usage.
Cisp has a simple policy about capacity. When a link hits 80 percent they upgrade it, if I remember one of Paul's posts correctly.
