I also never got used to using the mouse scroll with my middle finger. My pointing finger does both left-clicking and mouse scrolling whilst my middle finger only does right-clicking. I feel that my middle finger is too long to bend for mouse scrolling. The last two fingers never get used.

Your palm can still rest happily on your mousepad. I've never felt pain with the finger grip.

i have a razer naga for WoW, it forces me to use fingertips otherwise i cant hit all the buttons with my thumb.

I use my ring finger/pinky to push it into the fleshy part under my thumb, then use my thumb for balance/so that the mouse doesn't rotate out of my hand.

It gives better fine control as compared to fingers only, but is a lot more comfortable long term.
