PHP Poll Script

Hi

I run a small website - http://exams.morza.co.za that has mock pilot exams. I want to add a small poll in the menu asking users what they want next in the site. Is there any PHP script I can use for this where I can add the data to my current database instead of the script creating it's own one to store the results in?
