I have a headless HP microserver purely for content and running my Plex server.

What is the best setting for Power Saving mode?

I want it instantly available (I have an external that sleeps ... With a few second lag to wake up when you want to use it) - just HDD on?

USB, WLAN, sleep and all the custom settings are can be off?
 
What power saving options are available? I don't think Microserver is going itself into power saving mode. However many harddrives (including external) do when idle and nothing you can do about, I am afraid.

The model number of the drive might help, as some power saving features can be disabled (hard drive itself, not a make of the enclosure). Microservers have eSATA, use it if USB bridge is giving problems.

What OS?

If you want instant then Standby is really your only option.

Microserver is very light on power already anyway.
