In a nutshell, diffrent DSLAM sites, use diffrent gateways to get onto the Telkom grid network.
I'm litterally 20km from you, and you're using a diffrent Gateway than i am.
I'm using 41.242.0.1, and so is eveyone else in Brits. Our DSLAM is connected to PTA, which is lightning fast i must say. Sometimes over the weekends, and early mornings, i had single digit pings to telkom ip's, as well as in HL2DM
Pinging 41.242.0.1 with 32 bytes of data:
Reply from 41.242.0.1: bytes=32 time=18ms TTL=255

So, Harties is connecting to a diffrent dslam router, i.e. sandton/krugersdorp - or closet town to harties, but not PTA! This dslam could be overloaded, and thus giving you poor speeds. It could also be the line conditions, fibre has lots of collisions, copper might be old, etc...
The more data you send to a host, the higher the latency becomes, because ADSL is Asynchronous and not Synchronous, like SDSL. On SDSL you have a reserved block of data that gets send to the dslam each time.
