Edit: Article says truck had to brake suddenly to avoid a delivery vehicle which had turned in front of it at the T-junction, the chain securing the load snapped and it fell off.

I can understand the load breaking loose if there was a collision but if hard braking alone caused the load to break loose then it means the load was never secured properly in the first place.
I'm so glad I was not driving next to the truck when it broke loose.
