Yeh dude, I watched a women who had worked at the same plant for 12 years reverse off a dock.
They helped her out of the fork lift and asked what happened, she said she did not realize there was a dock there.

I told her supervisor that her inability to not use a cellphone while operating a dangerous piece of machinery was to blame and not her apparent loss of memory after 12 years of service on the same damn site.
The cameras verified my story and she was fired.

Countless times I had to jump a safety railing to avoid her.

Where was the supervisor while she was using a cellphone while operating the forklift? Looking the other way probably. Good that you got her fired though, too many idiots think that they can multi-task when they cannot. Particularly when operating dangerous/heavy machinery and vehicles.
