Using IAX2 protocol which is available on many third party SIP clients will probably fly under the radar as it is a tunneling protocol similar to a VPN.

Is this something that has to be built into the app? If VC or MTN decide to charge very high rates for VOIP traffic will you be bale to choose the protocol on say, Whatsapp, or does it have to be built into the app?

Id think it has to be built into the app. There are however VPN services available for smartphones.....

We have no idea what protocol or codec WhatsApp will use but there are many free SIP clients out there that also support IAX2 http://wiki.voip.ms/article/Softphones , take your pick
