First thing's first, you cut off the tongue and remove the vocal chords - you don't want a whiny patient, nor do you want them to be able to identify you later - this is important - read on. If I wanted to inflict pain though, I'd probably start with a few compound fractures of the femurs and collar-bones. I'd then erect the individual up so that they were held up but pressure was placed on the legs, with their arms bound in the air in lye-soaked ropes. The burn of the lye will keep the collar-bones constantly moving. Once you've achieved this you don't need to break any other bones - the pain will in no way compare. I'd surgically place a catheter directly into the recipient's liver - it might take a few attempts before I reach the vagus nerve, but once installed properly I could inflict copious amounts of pain and horrendous mental states at will without causing permanent damage. I'd cycle this on a weekly basis to allow the liver to repair itself slightly. During the weekly liver breaks, I'd inject them with enough LSD to place them in a suggestive mental state, and convince them that they have been rescued by the police and they are in the hospital. I believe a broken psyche is as damaging as physical pain.
I'd also kidnap his family and force him to make a choice between them being subject to the same treatment or death, with penalties for time wasting.
