Worthwhile VOiP providers

My understanding is that it also breaks provisioning from an external phone.

External phones shouldn't be connecting directly over the internet though. Better to setup a VPN and then make sure the external phones can connect to your instance over the VPN.

That is one of the things I don't like about 3CX...The phone provisioning is clunky. My opinion is that users should be users and not mess with settings at all, That is why IT support and VOIP support exist. Install Asterisk on a Linux server and you are good to go! It is very nearly my cloud hosted server's 6th birthday, Asterisk is rock solid!

SIP trunks are outbound connections only if they register to a host (user/pass, tells the host your IP basically). If the SIP trunk is IP authed then its an inbound connection and need firewall rules for SIP and RTP (calls get sent to your IP regardless of registration, there is no outbound registration).

Provisioning is also an inbound connection, also needs fw rule.

My wife is starting up her own practice and want to get her a dedicated number. I see 3CX now has a startup option which is free for up to 10 users. Now I guess I need to find a SIP provider. Who is the easiest/cheapest for this?

Saw that as well, but to use your own SIP trunk you need the startup pro, and while it has a get free button, says $175 a year. Which makes sense if they dont get any income off the calls.

However it also says they only host in these countries and SA isnt one https://www.3cx.com/support/hosting-service/

If you have a better idea how this works please explain as it looks good. Maybe start another thread

Happy to assist. We provide a 2ch trunk with a DID and 100 Mins included (SA destinations) for R59 / month. And hosted PBX options too (if managing an on prem device is an issue).

It looks like I can add a SIP Trunk in startup.
