Getting them to change the pair is easier said than done. Not even getting lawyers involved helped me. It was just cheaper and less stressful to get new lines

Next to impossible to achieve, its a big job to cut over a cable like that and Telkom don't wanna pay the overtime or expense of new cable.
Apparently the upgraded exchanges (ADSL 2+) don't work well with the crappy wiring, while the older ones (ADSL / G.DMT) were fine with it.

that it is indeed the exchange upgrades coupled with the poor copper lines which have been joined with different thicknesses in sections. The entire length needs to be replaced which could take forever.
