Probably best to focus on finding the issue on your network.

VoIP (requiring consistant/error free forwarding) is usually a good exposer of latent network inefficiencies, even though data transfer seems fine. The culprit is most likely an interface/link either running errors or being periodically saturated to max capacity.

Once you've sorted out the basics, then do your re-design for managability, flexibility, security etc.

Keeping vlans was a good idea. Also enable QoS on the 2960. I can send you a template if you want. You have to protect voice from data.

To sort out routing for the main site, create a static route for the pbx to point to the site router serial interface. The site router will know how to rote to the pqbx as its a directly connected interface and has inter-vlan routing setup (by means of sub-interfaces as you described).

What routing protocols are you running on your network?
 
Just read page 2 of this. You need to set up QoS by the look of things for your LAN and wifi link. DM me if you don't come right. I'll send you some templates with explanations
