Have you used the WC Public healthcare system?
Like everything else it all depends who is managing it, you have some clinics/hospitals that are really good (like private) and some that are not so good but none on the level of EC or NW.
When MO's come to wife's government clinic after internship (community service) in other provinces they think it is private because it is well stocked, clean and organized.
Doctors are dedicated and hard working, there is no nonsense (except occasionally with sisters) and it works quite well.
Unfortunately WC budget is cut by 1.6 billion (or around there) for this year and it feels but they still try to provide best, dignified care.
Main issue is increase in number of patients with budget that is not growing but it is cut.
Jan and Feb are well known for influx of patients from other provinces which also puts strain (not to mention violence).
Fiscal control is tight and there is not food, drinks during meetings, every Rand counts.
In my mind (also in medical field) there is no need for NHI but what we need is proper management of resources that we have in government sector, there is no reason for EC health to be so much worse than WC with similar budget.
