No I meant if someone updates the document, or form on Google docs, how would it be sent to the author?

Use Tools/Notifications in the GoogleDocs spreadsheet for email updates, etc. Btw, GoogleDocs spreadsheets can have pretty powerful uses - I've them running as generalised games/calculation engines for Facebook apps and Flash apps.

How many staff members do you have? If not too many the easiest is going to have a form for each one, check link

You can always make a HTML page with links to each staff member to make it easier to enter the data.

or one form with a drop down list with each staff member on the list
