What is your MTU size set to? Normally it should be 1492 bytes for PPPoE connections over ftth

For high packet loss, lower is usually better, so 1480 he has now is better, but doesn't really make a difference.
I'm in CT, FF, set to 1500 by default with DHCP/Dynamic IP Address for net.

Yeah, try a different Ethernet cable and plug directly in and see.
FF do have a device they can set up at a person's home address for them testing the line from their side, can always ask for it. You just need your ISP to open a ticket (and have to go through the entire support motions).
 
How to see what the maximum MTU size is:

Open CMD and enter the following:

ping -f -l xxxx www.google.co.za

The -f flag instructs ping to not fragment the ICMP request.
The -l flag followed by a value (in bytes) specifies the ICMP payload size.
www.google.co.za is the hostname

Start with a low number like 1450 and work your way up incrementing by 1 until the ping fails with the following error:

Code: 
Packet needs to be fragmented but DF set.

The last successful ping will be your maximum MTU size. Take the biggest successful packet size and add 28 to the value, to account for the TCP and ICMP headers. So if your maximum MTU size is 1464 as per the below example, your MTU size will be 1492 (1464 + 28 = 1492).

Is this with LAN? I'd probably try a different device, if you have one available, just to make sure you aren't fighting with a suspect LAN adapter or driver.

If it's WiFi, I see it often on my network as well and I have come to accept it as par for the course as you have all your devices fighting for transmit windows, that's not even considering neighbouring AP's that are also broadcasting and fighting for the 2.4 or 5GHz spectrum

It's lan.

I've got two WAN devices, both do it. Reset the one last night with same effect.
 
bypassed the router, plugged into the ont unit. Pinged the ip they gave me and still it had spikes of 160-170ms every 4-5mins.

Some packet loss now and then as well. The ip they gave me only had two hops. Ran CMD, Pingplotter and WinMTR all showing spikes. So i guess it shows the Cool ideas has a problem. They'll probably say it's the ONT ( it's been swapped twice this year :D )
