But the difference is that the term "best effort" (local translation: minimal effort) is used as a get out of jail free card here.

Oh, it's not just here - don't worry, that's a worldwide phenomenon. Check the Ookla index, there's a lot of countries with far lower "best efforts" than South Africa - including UK, Italy, France, Australia.
 
I think this article is about paying for a speed (example: 1Mb/s) and getting the speed (1Mb/s), averaged and compared between countries. It looks like you think it's the difference in cost and difference in ISP's........am I right?
Yeah I didn't address the article head on. For me its a simple question of value for money though...which includes speed among other things. i.e. Its the thread title interpreted differently:
