Kwaal hand - Meaning

I have seen this sticker of a hand with thumb and forefinger trying to pick something, with the words "Kwaal" underneath it. Can someone explain to me the significance of it? I've seen it on taxi's etc...

Okay, it is more tsotsi taal than it is Afrikaans, so it can safely be assumed that it has nothing to do with a disease in this context.

As @dabean linked, it is an afrikaans word meaning ailment. As in:
"Ek het verskeie kwale, onder andere vigs, aambeie en loopmaag. Dit is 'n vreeslike ingewikkelde gat siekte."

It has a different meaning in this context.
