Can anyone suggest a good place to start?

We have this book in the office, is this ok?

If you've only just realised that now then I'm afraid there's no book out there that can help you.

Don't talk nonsense. Everybody starts from a point of ignorance and learns through study. Now he knows the difference he is already more knowledgeable than before and with hard work and study his knowledge will continue to grow.
 
For Android, you may want to carefully weigh the option of using the new Android Studio instead of the traditional Eclipse route. It's easier to get going "out of the box" and seems to have the full weight of Google behind it.

Might be easier if you already are familiar with normal web development languages (HTML, JS, CSS) then maybe take a look at cross development tools like PhoneGap (http://phonegap.com/).

I've developed in HTML and CSS for some time so will check that out.

HTML and CSS are not procedural languages. They won't help you very much with either Objective C or Java. It's good that you can do them, but you need some basics with object oriented language coding, and that will take you quite a while to build up. Rather than starting with Android development then for instance, pick up a book on Java, or go even further back and get skilled up on coding basics.

None. You need a Mac. You can Google the details if you want more info.

Wrong. Phonegap allows you to build apps for any platform without having to use native SDK's.
