Proline tablet + Telkom mobile problem

I have a Proline tablet pc with windows 8.1, telkom simcard and want to go online but am not able to set the connection up. Van somebody help please?
 
What error are you getting?

If you need to setup the APN, use "internet" no need for username or password.
