I've had yearly tyre flats on the same position - rear passenger-side tyre.
One just happened this morning. This is the third year running in the same position.

I noticed an unusual wear pattern (exposed white material) on the inside edge of the tyre, here are some images:

The exposed white ring looks like a wear pattern from some object while driving.

The tyre is only a year old and as you can see the thread is still almost like new.
Does anyone have any idea what might be causing this?

Replace the air inside the tire with this:

1. The wrong tyre fitted to the vehicle.
2. Lowered suspension.
3. Vrot shocks
4. Underinflated tyre --- Rim damage, tyre going flat -- poor maintenance.
5. Overloaded vehicle.
6. Damage to the bottom of the vehicle.
7. Suspension damage or out of position.
8. Really bad driving over speed bumps --- you are supposed to slow down over speed bumps.
9. Oversize/wrong rims not suited for fitment to the vehicle
10. Maybe you should not own a vehicle or drive one? Can't possibly believe you did not hear a noise or feel an unusual vibration in all this time.

How on earth did you not look for the cause on the very first tyre change?
That is a very bad cut on the tyre.

/Edit: added the highlighted part as per @Colin62 's post thanks:D

If the same wheel is going flat, perhaps it’s the rim. It might be leaking air through a tiny crack, or not sealing 100% with the tyre.

That wear pattern you’re seeing on the sidewalk is from the tyre being driven once the tyre has gone flat. It’s a symptom, not the cause.

The first step is to check pressure every single time you fill up.

If it’s always the left rear that’s a little low, try swapping the front and rear wheels. If the problem follows the wheels to the front, it’s almost certainly the rim. If it stays on the rear, start looking for problems with suspension, shocks, alignment that type of thing.

I can’t see the tread across the width of the tyre - if it’s even and uniform then probably not a suspension issue.

It might be 3 and 8.
I think when I drive over the speed bump, the shock is making impact with the tyre and over time eroding the tyre.

First time I thought it was just a puncture, second time coincidence, third time - there's something wrong here.

I think it's unlikely a root caused that.

Looks pretty bad for it to be only from being driven flat. i think it might be rubbing on something on the inside of the wheel arch.

The shock mounting point stays in place, so i doubt its the shock rubbing when its compressed while going over bumps.

Put another wheel on then get your head in there and look. secure the car first of course.

Looks to me like your sidewall has been rubbing on something. There's a ring of abrasion all the way around the tyre directly over where your sidewall failed

Does anyone else see this?

Maybe it's rubbing on the wheel lining or a part of the body when you go around corners.

Have you noticed any odd noises from the car when going around tight corners? I know these are on the back, but maybe your camber is out or there is something loose on the suspension.

The flat tyre story also seems likely and would also create this ring

You could be right I drove about 2 km with it completely flat - I had no choice.

The thread is even.

I'll have someone take a look at the rim.

You'd hear it if something was rubbing.

There is always a choice! Where was your spare? The more you post the more convinced I am point 10 applies.
