Samsung Galaxy S6

For those missing quick toggles, Samsung has released an app to temporarily fix the problem:

Got the program in the Samsung app store, installed it, ran it and it then uninstalled itself as advertised ... but after a restart the quick panel shortcuts still don't reappear.

Did this actually work for anybody?

EDIT: Ok, got it to work. Turns out you have to click on 'EDIT' in the top-right of the quickpanel, then drag the 'Mobile Data' shortcut from the second group of shortcuts up to the first group again. Not terribly easy.

pull down the notification shade - press edit - it should all be there

Have to take wife's S6 in for repairs. Seems the camera module has failed. It will focus fine on objects about 5cm away, but completely out of focus for anything further away.
Tried Google Camera as well just to make sure it wasn't software related - same problem

Lens scratched?

Nope, no scratches. I checked that first.
No doubt it's the optical elements that have failed. Lenses are sensitive mechanical devices, and are more prone to failure than their software counterparts.

They should just swap out the device under warranty.

Doubt they'll swop it out. They had camera modules in stock, so they'll probably start with changing that.
They did try a firmware flash with no luck, so definitely hardware.
