Diet is super important, add fiber, add water.
I would definitely keep surgery way way way down the list of things to try, probably best to keep it off the list.
Coconut oil applied to the area can help. Definitely don't strain when on the toilet. If it feels like it's going to be a tricky one you can apply a bit of vaseline beforehand. If you absolutely have to use toilet paper then use just the gentlest wipe, but clean with water afterwards, you want to avoid any harsh rubbing and a lot of violent wiping with toilet paper is not going to help, cleaning with water is better.
As I understand, the pain can cause the sphincter to clench harder and cause more problems and slows down healing, relax if you can (easier said than done).
If it's a recurring problem you might find that becoming more intimate with the area, e.g. using toys or being on the receiving end of penetration can help you learn to relax and be more in tune with what's going on back there. I wouldn't recommend getting into anything crazy while you're still actively healing up though.
