You can use one account. The issue is you need to trick steam into thinking you are currently using the internet from the US.

so I can buy with a South African account? how about gifting, can I gift US games to a SA account? I might setup a US account to gift only if that's the case but payment may be tricky... paypal is SA account

Yes you can buy with an SA account. Certain specials/products might be restrict to certain regions in which case you can't buy unless you use a proxy into tricking steam you are accesing from the us/eu. Yeah you can gift to whoever you want, there's no restriction on owning just selling for certain products.

Steam doesn't restrict accounts they detect where you are accesing from.

and how do you fool the billing address ? no use having an us account and you billing is rsa...

Do they actually check the billing address?? If they do it would be an issue but from what I read/heard it does work. Obviously its risky and not really advisable but it has been done.

at the moment of finalizing the transaction the country you are doing the purchase form is checked against your billing address.
 
The reason why it can't be done is because of credit cards.. Credit cards are authorised via regions. Our credit cards will never work even if we spoof IP's..
Unless we get a US friend to gift it to us

Steam accepts Paypal, so i doubt they verify on credit card level .....
